Patient Notes

To access a patient's notes, first navigate to the Patient's Listing page. Locate the patient and click on the briefcase icon under Quick Actions.

An accordion will appear at the top of the page. By default, the patient's details are collapsed.

Once when clicked on the expand button, you can see the expanded Patient details as follows. On the left, you can also view a call icon beside the Mobile, which is used to call the patient on his cell.

Next, click on the Notes tab. An empty section will appear with a + Add Note button in the upper right corner.

Adding a New Note
Click the + Add Note button to open the notes form. Fill out the following fields:
- Date & Time: The time the notes were recorded.
- Mode of interaction: How the notes were taken. Options include phone call, sms, email, in-app messaging, face-to-face visits, and others.

- Notes: The main body of the note, for key discussion points or future reference.
- Service: The service related to the note. Options include TCM, CCM, RPM, AWV, and Others.

- Duration: The length of the conversation.
After filling in the required fields, use the action buttons at the bottom of the form:
- Save: Saves the note.
- Cancel: Clears the form and returns to the initial state.

Once the note is saved, it will appear in a record on the right side of the screen.
Editing and Managing Notes
After a note is saved, all fields are disabled. To make changes, click the Edit button. After editing, use the following buttons at the bottom right:
- Update: Saves the changes.
- Cancel: Discards the changes and returns to the disabled view.


You can add new notes by clicking the + Add New button, which provides a fresh form. This allows you to create multiple notes for a patient.

Using Call Templates
The system allows you to use call templates for active services. When you click on a service, a list of available templates will appear.

Click on a template to open a pop-up. You must answer at least one question in the template to submit it.

If your clinical timer is not active, the submitted template will be attached to a non-billable record, which you can view during clinical time.

- If your clinical timer is active, only the service for which the timer is running will be enabled. The submitted template will be attached to that billable record in the clinical time tab.
You can view the full details of a saved call template by clicking on View Details within a note's record.

You can also download each note by clicking the download icon.
