Patient Notes

To access a patient’s notes, please begin by navigating to the Patient's Listing page. From there, locate the patient and click the briefcase icon under Quick Actions..

An accordion will appear at the top of the page. By default, the patient’s details are collapsed.

After clicking the Expand button, you can view the expanded Patient details as follows. On the left side, you can also see a call icon next to the Mobile field, which is used to call the patient on his cell.

Next, click on the Notes tab. An empty section will appear with a + Add Note button in the upper right corner.

Adding a New Note
Please select the + Add Note button to open the notes form. Complete the following fields:
- Date & Time: Enter the time the notes were recorded.
- Mode of interaction: Indicate how the notes were captured. Available options include phone call, sms, email, in-app messaging, face-to-face visits, critical alerts, adverse event, provider escalation and others.

- Notes: The main body of the note, for key discussion points or future reference.
- Service: The service related to the note. Options include TCM, CCM, RPM, AWV, and Others.
- Note Author: Notes created by.

- Duration: The length of the conversation.
After filling in the required fields, use the action buttons at the bottom of the form:
- Save: Saves the note.
- Cancel: Clears the form and returns to the initial state.

Once the note is saved, it will appear in a record on the right side of the screen.
Filters:
You may filter notes to quickly locate specific records. To use the filter, click More Filters, and the available filtering options will appear below. You can narrow notes by entering note text, selecting a service, choosing a month, or selecting the mode.
Editing and Managing Notes
Once a note is saved, the record will be added to the list. To access any record, click the desired record. By default, all fields are disabled. To make changes, click the Edit button. After completing your edits, use the buttons located at the bottom right:
- Update: Saves your changes.
- Cancel: Discards your changes and returns you to the disabled view.


You may add new notes by clicking the + Add New button, which opens a blank form. This enables you to create multiple notes for a patient as needed.

Using Call Templates
The system enables you to use call templates for active services. When you select a service, a list of available templates will appear.

Please select a template to open the pop-up. To submit it, you are required to answer at least one question within the template.

- If your clinical timer is not active, the submitted template will be attached to a non-billable record, which you can access and review during clinical time.

- If your clinical timer is active, only the service for which the timer is running will be enabled. The submitted template will be attached to that billable record in the clinical time tab.
- You may review the full details of a saved call template by clicking View Details within the note’s record.

You may also download each note by clicking the download icon.

Note: If a note record is associated with adverse events or critical alerts, the record cannot be edited.
