Patient Survey
Note:
Survey is static, the patient will receive Survey to answer and these shared answers will be recorded and can be seen in the Survey Tab in the Provider portal.
To view Survey, click on the briefcase icon in the Patient's listing - Quick Actions.

You can see an accordion on top, it can be expanded and closed when needed. By default, the patient details accordion will be closed.

- And, once when clicked on the expand button, you can see the expanded Patient details as follows. On the left, you can also view a call icon beside the Mobile, that is used to call the patient on his cell.

- Click on the Survey tab, and you can see it has two sections which are Survey Answered and Send Survey.
Survey Answered
- In the Survey Answered Tab you can see the list of Surveys answered by the patient
- To view Surveys assigned to the patient. Surveys can be assigned based on the Patient Status and Period - Daily, Weekly and Monthly
- You can view the Calendar, on the top and on the left, you will see the Survey list. On clicking any of the survey will display the Patient's Survey and responded answers on the right.

Send Survey
- In the survey, we have three sections: Survey History, Survey List, and Send Survey by.
- In the Survey History, you can see the surveys that you have sent to the patient to answer. It includes the time and date, serving as a log history for future reference.
- In the Survey List, you can see all the surveys that you have added at the center level. All those surveys will be reflected here.
- To send the survey to the patient, select the survey from the survey list that you would like to send and choose an option (you can select single or both options) which are email and SMS. Then click on submit.
Once it is sent, you can see the history in the Survey History section.

- Surveys can be viewed on daily basis, you can select the period that you want to view the surveys from, by selecting the From and To date range in the Duration field at the top-right.
- You can click on the dates to view responses respectively.
